HIRSCH IH. Optimizing fertility potential in spinal cord injured men. Can J Urol 2012;19(5):6437. even after spinal cord injury. As the hurdle of semen retrieval has been successfully transcended in over 90% of such men, our research objectives should be directed toward the optimization of semen quality associated with neurogenic infertility. Over the past 3 decades significant therapeutic inroads have allowed spinal cord injured men to achieve biologic paternity. Prior to the introduction of electroejaculation in the mid-1980’s, men with spinal cord injury were considered “hopelessly infertile”. With the application of semen retrieval techniques such as electroejaculation (used to achieve the first reported pregnancy by a quadriplegic male)1 and penile vibratory stimulation2 these patients are now “potentially fertile”. Additionally, the coincident application of assisted reproductive technology to these couples has yielded pregnancy rates similar to those in able-bodied subfertile cohorts.3 Thus the field of Reproductive Rehabilitation has evolved to complement the traditional areas of Sexual Rehabilitation and Bladder Rehabilitation of spinal cord injured men. Along with the realization of these reproductive milestones, advances have also progressed in the description of spermatogenesis4 and semen quality following spinal cord injury.5-8 This article by Leduc is emblematic of the spectrum of effort during the development of Reproductive Rehabilitation. The authors have applied various medical, technical and surgical methods with and without assisted reproductive techniques to optimize pregnancy rates to 64% of their spinal cord injured population.9 These results and those of other centers dedicated to the quality of life of spinal cord injured men are testimony to the fertility potential of men References

ELECTROEJA CULATION FOR FERTILITY IN MEN WITH SPINAL CORD INJURY

From October 1990, to June 1992, more than 800 electroejaculations and 108 intrauterine inseminations (IUI) were performed in 250 paraplegic males and their wives. Seven successful pregnancies are the results of this 20-month effort. This is the first report of electroejaculation and successful pregnancy with its use in paraplegics in Iran and the Middle East.

Penile vibratory stimulation and electroejaculation in the treatment of ejaculatory dysfunction.

The purpose of this review is to present the current understanding of penile vibratory stimulation (PVS) and electroejaculation (EEJ) procedures and its clinical use in men with ejaculatory dysfunction. Unfortunately, the record of treating such individuals has been quite poor, but within recent years development and refinement of PVS and EEJ in men with spinal cord injury (SCI) has significant...

Compound Action Potential of Isolated Spinal Cord: A Biophysical Analysis to Address Activity of Individual Fibers Following Contusion Injury

Compound action potential (CAP) of spinal cord represents valuable properties of neural fibers including excitability, rate of myelination and membrane integrity. These properties are measured using amplitude, latency and area under curve of CAPs recorded from spinal cord. Here, the isolated spinal cord was set in a double sucrose gap (DSG) chamber and its response to intracellular stimulation ...

Effects of Biodegradable Polymers on the Rat's Damaged Spinal Cord Neural Membranes

The overall goal of this study was to identify the appropriate biomaterials able to facilitate the regeneration in rat's injured adult spinal cord. Acute damage to axons is manifested as a breach in their membranes, ionexchange distortion across the compromised region, local depolarization and even conduction block. It would be of particular importance to interrupt the progress of events h...
